-ectomy, -tomy, -stomy, -plasty: The Surgical Suffixes Compared

Four endings differ by a couple of letters and describe four entirely different operations. -ectomy takes something out. -tomy cuts into it. -stomy makes a lasting opening. -plasty repairs or reshapes it. Read the ending wrong and you have understood the wrong procedure.

This is the most consequential set of look-alikes in medical vocabulary, and it is also one of the most learnable, because the distinctions are clean and the suffixes attach to roots you already know. This guide sets the surgical suffixes out side by side, explains the -tomy family's confusing triplet, and covers the seven less common procedural endings that round out the set.

The big four, in one table

Start by fixing the four core meanings in a single mental image. Each one answers a different question about what happened to the structure named by the root.

-ectomy: it was removed. -tomy: it was cut into. -stomy: an opening was created. -plasty: it was repaired or reshaped. Notice that only -ectomy begins with a vowel, which means it is the only one of the four that drops the combining vowel in front of it - appendectomy and nephrectomy, but osteotomy, tracheostomy and arthroplasty.

  • -ectomy - surgical removal. splen/o + -ectomy = splenectomy; cholecyst/o + -ectomy = cholecystectomy.
  • -tomy - incision, cutting into. crani/o + -tomy = craniotomy; lapar/o + -tomy = laparotomy.
  • -stomy - creating an opening. ile/o + -stomy = ileostomy; trache/o + -stomy = tracheostomy.
  • -plasty - surgical repair or reshaping. arthr/o + -plasty = arthroplasty; angi/o + -plasty = angioplasty.

The -tomy triplet: -tomy, -otomy, -ostomy

Reference lists, TermDecode's included, carry -tomy and -otomy as separate entries, and likewise -stomy and -ostomy. That looks like duplication and confuses a lot of learners, so it is worth being clear about what is going on.

The underlying suffixes are -tomy (incision) and -stomy (opening). The forms with an extra o at the front are the same suffixes written with the combining vowel already attached, which is how they most often appear in finished words. Because most roots that take these suffixes end in a consonant, the vowel survives, and the visible ending is -otomy or -ostomy: craniotomy, osteotomy, episiotomy, laparotomy, ileostomy, colostomy, tracheostomy.

The practical rule is simple. Read the final -tomy or -stomy as the meaningful unit and treat the o in front of it as the combining vowel doing its ordinary job. Do not read -otomy as if it were a third, separate suffix with a meaning of its own.

The distinction that actually matters is the s. -tomy is a cut; -stomy is an opening that is meant to persist. One letter, two very different procedures, and it is the single most valuable letter to read carefully in the whole surgical vocabulary.

-ectomy is not the same as -tomy

The second confusion worth heading off is between removal and incision. Both endings contain -tomy - the Greek root behind both is about cutting - but -ectomy carries the additional ec- element meaning out.

So gastrectomy is removal of the stomach, while a term in -tomy on the same root would be an incision into it. Osteotomy is cutting bone; there is no removal claimed. Craniotomy is incision into the skull. Reading -ectomy where the word says -tomy converts an incision into an amputation, which is exactly the kind of error that a careful reading of the last six letters prevents.

There is a helpful spelling tell. -ectomy begins with a vowel, so it strips the combining vowel: you will see nephrectomy, splenectomy, appendectomy, gastrectomy, thyroidectomy - root, then straight into -ectomy. -tomy begins with a consonant, so the o survives: osteotomy, craniotomy, laparotomy. If you can see an o at the join, you are almost certainly looking at an incision rather than a removal.

The seven that complete the set

Beyond the big four, seven more procedural suffixes appear often enough to be worth learning in the same session. Each names a specific technique rather than a general category.

All seven begin with a consonant, so all seven keep the combining vowel. That gives you a useful consistency: within the surgical group, -ectomy is the only ending that strips the vowel.

  • -centesis - surgical puncture. thorac/o + -centesis = thoracentesis; the dataset also lists amniocentesis.
  • -rrhaphy - suturing. herni/o + -rrhaphy = herniorrhaphy, with neurorrhaphy alongside it. Note the double r, which several Greek suffixes share.
  • -pexy - surgical fixation. The dataset's examples are nephropexy and orchiopexy.
  • -desis - surgical fusion. arthr/o + -desis = arthrodesis; spondylodesis is the other listed example.
  • -tripsy - crushing. lith/o + -tripsy = lithotripsy, from the root for stone.
  • -clasis - breaking. oste/o + -clasis = osteoclasis.
  • -ligation - tying or binding, listed with tubal ligation as its example.

Procedural suffixes that are not surgical

A second family sits next to the surgical set and is frequently mixed in with it, because both describe things done to a patient. These name looking and recording rather than operating, and keeping them separate is worth the small effort.

-scopy is visual examination, and -scope is the instrument that does it: arthroscopy, cystoscopy, endoscopy, bronchoscopy. -graphy is the process of recording, -graph the instrument, and -gram the record produced - mammography is the process, an electrocardiogram is the result. -metry is measurement and -meter the measuring instrument. -opsy is viewing, as in biopsy.

The instrument-versus-process-versus-result distinction is the one to drill here. -graphy, -graph and -gram all come from the same root and differ only in what they point at: the doing, the device, the output. Once that triple is clear, a large slice of diagnostic vocabulary reads itself.

Drilling the look-alikes

Because these suffixes differ by so little on paper, recognition drills are close to useless - everything looks plausible when it is in front of you. Production drills are what work.

Fix a root and rotate the suffix. Take arthr/o and write out the repair, the fusion, the visual examination and the pain terms - arthroplasty, arthrodesis, arthroscopy, arthralgia, all four of which the dataset carries. Take gastr/o and do the same. You are practising the contrast rather than the item, and you are also practising the combining-vowel decision at every step, since some of those endings strip the vowel and some keep it.

Then go the other way. Given a plain-English description - surgical creation of an opening from the ileum, surgical repair of a joint, surgical puncture of the chest wall - produce the term. Finally, read the last six letters of every procedural term you meet, deliberately and slowly, for about a week. The habit generalises quickly and it is the single behaviour that prevents almost every error in this group.
