Medical Terminology Plurals: The Latin and Greek Endings, TermDecode

Most of medical vocabulary is assembled from prefixes, roots, and suffixes, and once you know the parts you can build the word. Plurals are the exception. They are not built from word parts at all - they are inherited whole from Latin and Greek grammar, and they follow the rules of those languages rather than the rules of English.

That is why one diagnosis becomes two diagnoses rather than diagnosises, why one bacterium becomes bacteria, and why a single vertebra becomes vertebrae. There are only about eight patterns worth learning, they map almost perfectly onto the singular ending, and after an hour of deliberate practice they stop being a source of error.

Why medical plurals are not English plurals

English forms plurals by adding -s or -es to almost anything. Latin and Greek inflect the ending itself, changing the final syllable rather than appending to it, and medical vocabulary borrowed the nouns together with their inflections.

Two consequences follow. First, the plural is predictable from the singular ending, not from the meaning - you do not need to know what a bursa is to know that two of them are bursae. Second, the plural often does not sound like the singular plus a hiss, which is why medical plurals are more often mispronounced than misspelled.

It also helps to know which language a noun came from, because the language determines the pattern. Endings in -us, -um, and -a are typically Latin. The Latin patterns: -us, -um, -a

Three Latin noun classes cover a large share of anatomical vocabulary, and each has a single, reliable plural.

Singular -us becomes plural -i. One bronchus, two bronchi. One alveolus, two alveoli. One thrombus, two thrombi. One nucleus, two nuclei. The plural is pronounced either as a long i (BRON-kye) or as ee (BRON-kee) depending on region and teaching tradition; both are heard, and neither is wrong in itself, so follow whatever your course or workplace uses.

Singular -um becomes plural -a. One bacterium, many bacteria. One ovum, two ova. One atrium, two atria. One septum, two septa. This is the pattern behind the most widespread plural error in English generally: bacteria and data are plural, and a single one is a bacterium and a datum.

Singular -a becomes plural -ae. One vertebra, two vertebrae. One bursa, two bursae - the structure named by burs/o, the root behind bursitis. One pleura, two pleurae. The -ae ending is usually pronounced ee in clinical speech (ver-tuh-BREE), though ay is also taught; again, match your setting.

The airway root bronch/o gives bronchitis; the noun itself pluralises the Latin way.

bacterium → bacteria

Latin -um → -a

One organism is a bacterium; bacteria is already plural.

vertebra → vertebrae

Latin -a → -ae

The Greek patterns: -is, -on, -ma, -nx

Greek contributes the other half of the system, and its patterns are just as regular once you can spot them.

Singular -is becomes plural -es, pronounced eez. One diagnosis, two diagnoses (dy-ag-NOH-seez). One prognosis, two prognoses. One metastasis, two metastases. One anastomosis, two anastomoses. The written change is small - a single letter - but the spoken change is large, and mispronouncing diagnoses as if it rhymed with roses is a common giveaway.

Singular -on becomes plural -a. One ganglion, two ganglia - the structure behind the root gangli/o. One spermatozoon, many spermatozoa. One phenomenon, two phenomena.

Singular -ma becomes plural -mata in strict usage. One stoma, two stomata. One carcinoma, two carcinomata. In practice, English plurals have largely taken over here, and carcinomas, melanomas, lipomas and lymphomas are entirely standard in current clinical writing. Recognise -mata when you meet it; you will rarely need to produce it.

Singular -nx becomes plural -nges, with the g softening to a j sound. One phalanx, two phalanges (fuh-LAN-jeez) - the bones behind the root phalang/o. One larynx, two larynges, from laryng/o, the root in laryngitis. One meninx, three meninges, from mening/o, the root in meningitis. That last one is worth noticing: meninges is far more common than the singular, which is why meningitis is built on the plural form.

dia- (through) + gnosis (knowledge); the plural changes only one letter but shifts the stress and the vowel.

The finger and toe bones named by phalang/o.

The -ex and -ix pattern

A smaller Latin class ends in -ex or -ix and takes -ices in the plural. One apex, two apices. One cortex, two cortices. One index, two indices. One appendix, two appendices.

This class is where English competition is fiercest. Appendixes and indexes are both in use, and in general writing they are unremarkable. Anatomical and clinical writing tends to keep the classical forms, and appendices is the form you will see in an anatomy text. Cortices and apices have essentially no English rival and should always be used.

The pronunciation shift is worth rehearsing: cortex has stress on the first syllable, and cortices moves to KOR-tih-seez. The plural does not simply add a sound to the singular; it re-forms the ending.

When English wins

Classical plurals are not a matter of correctness for its own sake, and a rule that says always use the Latin form will make you sound stilted rather than precise. Usage has genuinely shifted in several classes.

The -oma family has almost entirely gone over to English: melanomas, lipomas, lymphomas, sarcomas and carcinomas are standard. Virus follows English rather than Latin - viruses, never viri. Sinus follows English too. Fibromas and hematomas are normal; fibromata and hematomata read as archaic.

The safest working rule is to use the classical plural where the singular is a classical noun with no English-shaped competitor (vertebrae, bacteria, diagnoses, phalanges), and to follow local convention where both forms are current (appendices or appendixes, formulae or formulas). Your course materials, your institution's style guide, and the journals in your field are the authority for those; the point of learning the patterns is to recognise every form you meet, not to enforce one.

Where a plural genuinely matters - a chart, an exam answer, a piece of documentation - check rather than guess, exactly as you would check a term's definition rather than trusting a parse.

  • Keep the classical plural: vertebrae, bacteria, ova, diagnoses, metastases, phalanges, meninges, cortices, ganglia.
  • English has won: melanomas, lipomas, lymphomas, carcinomas, viruses, sinuses, hematomas.
  • Both are current: appendices and appendixes, formulae and formulas, indices and indexes.
  • Never mix: one bacteria and a criteria are the two errors most likely to be noticed.

Then reverse the drill, which is the version that actually matters in reading. Given a plural you have met in a text, work backwards to the singular so that you can look it up. Seeing pleurae and knowing to search for pleura, or seeing apices and knowing to search for apex, is the practical skill this knowledge buys you.

How to practise plurals without a textbook

Plurals reward a different kind of drill from word parts. Word parts are learned by decomposition; plurals are learned by transformation, so the practice has to involve producing a changed form rather than recognising a fixed one.

Three exercises are enough. Take twenty anatomical nouns you already know and write both forms side by side, saying each aloud - the spoken form is where the memory actually lives for -es and -nges words. Next, take a paragraph from any course text and underline every plural, then recover the singular from it. Finally, when you meet a new noun in the library, note its ending class in the same breath as its meaning, because the class is a one-word annotation that saves you a lookup later.

Pair this with the word-part work rather than treating it as separate. A term like meningitis makes far more sense once you know that meninges is a plural in -nges and that mening/o is built on it; a term like bursitis is easier to hold when you know the singular bursa and its plural bursae. The morphology and the inflection reinforce each other.
