Medical Prefixes: The Position, Number, Time and Degree Families
Prefixes are the smallest of the three word-part groups and by some distance the fastest to learn. TermDecode's dataset holds 68 of them against more than 250 roots, and most come in oppositions - above and below, fast and slow, before and after - which means you can learn them two at a time.
They are also the part where a single misread letter does the most damage. Swap hyper- for hypo- and the term means the opposite of what it says. Swap brady- for tachy- and the same thing happens. Roots and suffixes rarely invert a meaning; prefixes do it routinely.
This guide sorts the prefixes into five families, drills the dangerous pairs, and covers the small set of prefixes that change their own spelling. TermDecode is for education only. What a prefix does and does not do
A prefix sits at the front of a term and modifies everything after it. It never changes what kind of word the term is - that is the suffix's job - and it rarely changes the subject, which is the root's job. It adjusts: where, how many, when, how much.
Two structural facts follow, and both make prefixes easier than roots. First, prefixes attach directly, with no combining vowel inserted: tachy- plus cardi/o plus -ia gives tachycardia, and hyper- plus glyc/o plus -emia gives hyperglycemia. There is no join rule to remember. Second, a prefix is optional. Plenty of well-formed terms have none - gastritis, nephrology, arthralgia - so the absence of a prefix is not a sign that you have mis-parsed anything.
A prefix can also carry a whole term on its own when paired directly with a suffix. Polyuria is poly- plus -uria, with no root at all. Anemia is an- plus -emia. Dyspnea is dys- plus -pnea. Recognising this shape saves you from hunting for a root that was never there.
Family one: degree and quality
This is the family that appears most often in condition vocabulary, and it is the one to learn first.
hyper- means excessive or above normal; hypo- means deficient or below normal. They generate matched pairs throughout the library: hypertension and hypotension, hyperglycemia and hypoglycemia, hyperthyroidism and hypothyroidism. eu- means good, normal or well, and is the quiet member of the group - a euthyroid state is a normal one. dys- means difficult, painful or abnormal, giving dyspnea and dysphagia. mal- means bad or abnormal.
Also here: a- and an-, both meaning without or absence of, which produce apnea, anuria and anemia; and olig/o and oligo-, meaning few or scanty, behind oliguria. poly- means many or much, giving polyuria and polydipsia. Learning poly- and oligo- as an opposition, alongside a-/an- as the zero case, gives you a three-point scale that reads a large slice of the vocabulary.

Family two: position and direction
The positional prefixes are the largest family and mostly come from Latin. They are best learned in spatial oppositions, and it helps to picture each one rather than translate it.
Inside and outside: endo- (within), ecto- and exo- (outside), en- (in, within), ec- (out). Layers and levels: epi- (upon, above), sub- (under), supra- and super- (above), infra- (below). Relations between structures: inter- (between), intra- (within), extra- (outside, beyond), trans- (across, through), circum- and peri- (around). Direction: ab- (away from), ad- (toward), retro- (backward, behind), pro- (before, forward), dia- (through, across).
The library shows them at work: subcutaneous is sub- plus cutane/o plus -ous, beneath the skin. Pericarditis is peri- plus cardi/o plus -itis, inflammation of the membrane around the heart. Endoscopy is endo- plus -scopy, examination from within. Transdermal is trans- plus derm/o, across the skin.
para- deserves a note. As a prefix it means beside, near or abnormal, and TermDecode groups it on one page with the root para, because they are different entries that share a written form. Paraplegia uses it in front of -plegia.
Family three: number and quantity
The number prefixes are the easiest of all, because most of them survive in ordinary English and you already know them.
mono- and uni- (one), bi- (two), tri- (three), quadri- (four), multi- (many), poly- (many, much), hemi- and semi- (half), pan- (all, entire), nulli- (none), primi- (first).
They show up in a predictable set of shapes. Hemiplegia is hemi- plus -plegia, paralysis of half; quadriplegia is quadri- plus -plegia. Bilateral and unilateral differ only in the number prefix, bi- against uni-, and the dataset lists them as the example words for each. The counting prefixes nulli- (none) and primi- (first) sit in front of para, the root glossed as to bear, giving nullipara and primipara, with nulligravida and primigravida alongside them.
The only trap worth flagging is the Greek and Latin overlap: mono- is Greek and uni- is Latin, and both mean one. Which one a term uses depends on the language of the parts it was built from, not on any difference in meaning. The same holds for hemi- (Greek) and semi- (Latin).
Family four: time and sequence
A small family, and the least error-prone, because English has borrowed nearly all of it wholesale.
pre- and ante- mean before; post- means after; neo- means new. Prenatal and postnatal are pre- and post- on nat/o, the root for birth; neonatal is neo- on the same root. Postoperative is post- applied to a whole word rather than a classical root, which is common with this family.
Two nearby prefixes are not really temporal but often get filed here. meta- means change, beyond or after, and appears in metastasis and metabolism. pro- means before or forward, and gives prognosis - pro- plus gnosis, knowledge before the event. Keeping meta- and pro- next to the temporal set is a reasonable filing decision as long as you remember that both also carry non-temporal senses.
Family five: the rest worth knowing
A handful of prefixes do not sit in any of the four families but appear too often to leave out.
anti- (against, opposing) and contra- (against, opposite) give antibiotic, antigen, contraindication and contralateral. Note that they are not interchangeable: anti- is Greek and tends to mean acting against, while contra- is Latin and tends to mean on the opposite side. pseudo- means false, and gives pseudocyst. iso- means equal or same. macro- and micro- mean large and small; mega- means large or great; ultra- means beyond or in excess, as in ultrasound. re- means again or back, and de- means down, from or removing.
syn- and sym- mean together or with, and the alternation between them is purely phonetic: sym- appears before b, m and p. That is a spelling rule rather than a meaning difference, and it belongs with the shape-changing prefixes covered next.
Prefixes that change their own shape
A small number of prefixes alter their final letter depending on what follows, and knowing this prevents you from treating two spellings as two different prefixes.
a- becomes an- before a vowel. That is why it is apnea and aseptic but anemia, anuria and anoxia. TermDecode lists a- and an- as separate dataset entries because both forms appear in real terms, but they are the same prefix doing the same job.
syn- becomes sym- before b, m and p, for ease of pronunciation. in- and im- alternate the same way, as do co- and con-.
None of this changes meaning, and none of it is a combining vowel - the prefix is not gaining a linking letter, it is adapting its own final consonant to the sound that follows. If a term looks like it has an unfamiliar prefix, check whether it is a familiar one wearing its alternate spelling before you go hunting.
- a- / an- : an- before a vowel. apnea, aseptic vs anemia, anuria, anoxia.
- syn- / sym- : sym- before b, m, p.
- in- / im- and co- / con- : the same phonetic alternation.
- None of these insert a combining vowel; prefixes never take one.
Drill the pairs that invert meaning
If you spend one focused session on prefixes, spend it on the four pairs that reverse a term's meaning when misread. Everything else in this guide can be picked up in passing; these cannot.
hyper- and hypo-. brady- and tachy-. hyper- and hypo- again, in a different context, because glycemia and tension and thyroidism each generate a pair and they are easy to mix across contexts. And a-/an- against poly-, the zero and the excess ends of the same scale.
Drill them aloud rather than silently. The two syllables hy-per and hy-po are far more distinct to the ear than the written forms are to a tired eye, and the same is true of brady and tachy. Then drill in production: given a blood condition of deficient sugar, write the term; given a slow heart rate, write the term. Getting the prefix from a meaning is a different, harder skill than recognising it on a page, and it is the one that transfers.
